How do I fade the scars left behind from pimples?

- From Everyone Commenting On Last Week’s Post.

Dear ECOLWP, allow me to correct you before we proceed.

“How do I fade the POST INFLAMMATORY PIGMENTATION left behind from pimples?”

That’s better!

Because it isn’t a scar. It’s post inflammatory pigmentation, which is a show-offy way of saying the discolouration of skin due to the production of melanin after inflammation. In this case, we’re referring to the site of a pimple you’ve picked at, or was VERY persistent and hung around for ages, and has now left a reddish, browny or purpley mark. Cool.

Bottom line: melanin is what is causing the mark.

What you are actually dealing with here is hyperpigmentation, the same stuff that sprinkles and spreads across your upper lip, forehead and cheeks post-summer thanks to the sun, (adorable!) or after pregnancy or the pill thanks to hormones, and creates uneven skin tone and dark spots on the face (fun!).

As you can imagine now that you know this fancy fact, treating it as you would a traditional scar (with vitamin E, bio or rosehip oil, for example) won’t work. Yes, they heal and fade traditional, actual scars, but like I said, what you’re treating is not a scar. Keep using these oils, obviously, because they’re gorgeous, but don’t expect them to do much on those purple/red marks that refuse to buzz off.

What you need to be doing is the same stuff you would to fade pigmentation anywhere else: with specific whitening and brightening products. These work to break up and disperse the melanin, and therefore reduce the appearance of the browny-purpleness, and fade it to the point of not needing to use concealer on it every morning, as someone who shall remain nameless (“Zoe Blanche Foster”) has to do with the marks on her chin each day. They also give a terrific all over brightness to the skin, as the term ‘brightening’ suggests. For those too shy to ask, no, there is no bleaching involved.)

Here’s a plan of attack:

Cleanse with a brightening cleanser. This makes more difference than you would expect, as the melanin that has started to come out on my face – this is normal, it can become a bit darker before it, uh, “falls off” – this week proves. I like Priori’s CoffeeBerry cleanser and Dermalogica’s ChromaTRX White Tri-cleanse.

Use an all-over brightening product at night to fade the marks, and get some impressive luminosity and even skin tone, (I firmly believe it is THIS, not reducing wrinkles, that makes you look younger and more glowy), and then do a bit of targeted prodz-work on the marks as well.

So. Use either a serum (under your night cream, on cleansed skin) such as:

- Murad Age Spot and Pigment Lightening Gel

- Clinique Even Better Clinical Dark Spot Corrector

- Caudalie Vinoperfect Radiance Serum

- Ultraceuticals Eventone Sensitive Serum

Or bump your night cream and switch to one of these for a while:

- John Plunkett’s Superfade face cream. (It has 2% hydroquinone – the most effective skin lightening ingredient available – which is the strongest amount you can get over the counter, although you can get 3-4% by prescription. Be careful if you have sensitive skin; can be irritating.)

- Ultraceuticals Ultra C diminish Cream (strong! Beware sensitive souls)

- Shiseido White Lucent Brightening Moisture Cream (the matching serum is pretty fantastic also)

After doing this, I also recommend taking some strong brightening cream (such as the Superfade accelerator or a prescription hydroquinone cream) and dabbing a little bit onto any residual pimple marks for a very strong and targeted hit. Clever you!

Happily, all of this effort will do more than just fade your pimple marks, it will give you devastatingly glowy, eventoned skin, which is probably the best side-effect a dame could ask for, am I right? Or AM I LEFT? NO! I am right. Definitely right.

Fun facts that don’t fit anywhere else on this post:

1. There are also more intense and expensive options for those with severe pigmentation including having a program of IPL, Fraxel Re:store Dual, or Helios II Laser Toning, but that’s a whole other post.

2. Brightening prodz tend to be quite active in order to do their biz. It’s just how the Mars Bar melts, I’m afraid. Hydroquinone has an especially bad rep. The Shiseido and Clinique gear will suit sensitive types.

3. This stuff takes time to see results. Worth it. Stick with it, sweet ladybeetles! Please stick with it. And winter is the best time to start as you’re less likely to be in the sun. (According to traditional myths about winter and summer, but living in Sydney they feel more and more like fairytales, these “seasons.”)

And finally, what’s a Zoë post with out some ranting about sunscreen!!!!

Wearing sunscreen every is crucial if you’re brightening your skin. There is ZERO point spending cash and energy on brightening prodz to fade your melanin if you are just going to go in the sun and have UV re-ignite all of it all over again. (Especially since a lot of brightening ingredients – AHAs, kojic acid, retinoids, vitamin C, and hydroquinone – make your skin more susceptible to sun damage.)

    Thanks heaps for this enlightening (pardon the pun) piece of information. Post baby face is gonna get a make over with some brightening cleansers for sure. My skin is so wack though, I bit like “teenage” skin with some wrinkle coming through around the eyes….32 is a weird age. But I’ll take it as it come. Can’t wait to see the results.

    hi Ruth! i desperately need your help. I’m 17 years old and after having my deal with acne, i now have the red marks left behind on my cheeks. i have literally tried everything. I’m from australia, so getting my hands on some beauty products that help might be hard.. but, i’ve done bio oil, home remedies such as lemon and bi-carb soda, etc… microderm is much too expensive for me. today I’ve been doing my research on ‘john plunkett super fade skin cream’ for the face, which contains hydroquinone and claims to lighten the skin and remove any marks. should i trust this product? I’m getting so desperate. i really need to get rid of these marks in a couple of months and this is my last resort. does anyone else have any ideas? has anyone used this? apparently some people have had very red skin after using? PLEASE, PLEASE HELP!

    Please can someone help me. I have olive/dry skin and hyperpigmentation all over my cheeks and upper lip. Which product would you recommend? Im so embarrassed about my skin, it gets me down every day.

    I love Clinique even better dark spot corrector. I have been using it for about 3 months and noticed results after 1-2 weeks. It is expensive but the even glowy skin tone makes it very worth it. It also means I don’t wear makeup as often. Adore!

      Everything I am finding on the web says it’s banned here, in Japan and in the EU because it is linked to cancer, skin hardening & darkening, and genetic abnormalities from extended use. But I can’t find anything that looks legal and official describing the ban in Australia or when it happened etc, so I don’t know. I would probably steer clear of it, the Ultraceuticals Even Skinton Serum works for me….

    Hi Zoe,
    I was wondering if there are similar products that would work on arms and legs as well? I had a VERY bad bout of stress rash (like psoriasis) that persisted for 6 months last year, now its mostly all gone but I have “scars” everywhere. I am looking into laser, but not sure which ones to choose. But if there’s a cream I can use for my body like the ones you suggest for the face, I’d love to know.

    Also, I do have a lot of pigmentation on my face. My face was clear and smooth until my 3rd trimester of my pregnancy, when half of my face was suddenly covered in dark pigmented patches. Now my daughter is 3 its faded about 70% but I agree with you that it makes me look older than wrinkles do. I’m a busy Mum and tired of the daily foundation and concealer routine… if you could do a post explaining the different kinds of laser treatments and where to go for them, I’d be eternally grateful. I don’t mind spending money but on what I’m not sure… there’s so much out there that I’m totally confused!

    I don’t have a problem with acne pigmentation but I DO have a whole lot of yucky broken capillaries caused by pregnancy. Does anyone have any advice on those?

      Go to a dermatologist-run laser clinic. Laser or medical-grade IPL will get rid of those suckers. I’ve had mine done.

          I had a course of IPL for broken capillaries a few years ago as my skin was too delicate for laser. I seem to remember having about 6 treatments for about $600. It worked wonders! My skin was AMAZING! Two kids later I will be due to spoil myself again once I go back to work next year :)

    Fabulous as always, Zoe! I’m so glad you mentioned that pigmentation can get worse before it “falls off”; it’s almost as though you were in my bathroom a couple of nights ago listening to me thinking: grumble, grumble “I thought this serum was meant to clear up my pigmentation, not make it worse!” grumble grumble. (perhaps you could mention this in other articles where you wax lyrical on brightening products? Or more likely, you already have and I’ve missed it!) I will persevere with my brightening serum, thanks!

    I’ve been using Ultraceuticals Even Skintone Serum (not the sensitive version) after hearing Zoe rave about it in Amazing Face, Primped, etc. Have been using it every night for the last 8 weeks and have been getting amazing results. Still have some scarring (posta-acne and pigmentation from pregnancy) but they are definitely lighter, my skin is looking brighter, clearer and more glowy and I’m venturing out make-up free most days. I’ve also been using the Dr Dennis Gross Alpha-Beta Peels 3-4 mornings a week and finding great results. I figure it’s cheaper than me going to get facials. My skin is usually quite sensitive but I haven’t had any problems with either of these products (I did what was recommended and used it only a couple of times a week at first rather than daily to see how it would go) and I notice a difference if I don’t use them for a few days. Like what Zoe said – sun screen sun screen sun screen!!

        I buy them from Mecca, the stronger version seems totally fine on my sometimes sensitive skin. You get a 2 jars, with 2 different peel pads that you use step 1 first and obviously step 2 second :)

    I have this crazy bad on one cheek especially!
    I have actually found Alpha Beta Peels to be the best! I have really sensitive skin and rosacea and it is nice (and good for) both these conditions.
    Its not invasive or craaazy or anything (no, they dont peel off your face) its like a super awesome facial…and it rocks my world.
    It costs me $150 p. visit but there is cheaper out there (like $100).
    I also use a mineral makeup to stop more pimples and therefore pigment marks appearing! I use Laura Mercier which is also (yay!) SpF15.

    Thanks for a great post again, Zoe. I’ve been using Ultraceuticals Eventone Sensitive Serum morning and night for a while now (since you recommended it on Primped?) with great results, and have noticed even greater difference now I’m also using Ultraceuticals Clearing lotion (from the special acne treatment range they released recently-ish) after pottering for a bit; doing hair, clean teeth etc after patting the serum in, to give it time to absorb before adding the next layer!

    My question for you is.. probably a VERY dingus-y one… but.. does the clearing lotion count as a moisturiser too?
    With “lotion” in the title makes it too vague to figure out in my pretty little, unknowledgable head!

    My skin hasn’t been particularly dry since I started using the Ultraceut lotion in place of Burt’s Bee tea tree moisturiser.. Sometimes the concealer I use to spot cover (clinique anti-blemish one) goes a bit crusty (urgh!) by lunch time, which I don’t think it used to. But haven’t had dryness elsewhere.
    Should I still use a night cream in addition to eventone serum and clearing lotion?

    Oh! and I also changed out my old primer for the Dermalogical mattifying clearing gel – makes my skin like SILK! Completely heavenly!!! and only spot conceal, use mineral powder/ blush / highlighter combo I’ve used for donkeys!! and have had heaps of comments about how glowy I have been of late! All because of the sage advice from you here and in Amazing Face (aka: BIBLE!)

      Mimi – I’m using Be In Awe Lightening Serum to great effect ( after an almighty deep breakout after trying the oil cleansing method that ended up with an all over cystic infection that took double dose antibiotics to clear after which I was pretty blemish scarred all over) I’ve been using the Be In Awe since start of Geb every night and ive still have the bottle left, I would say I e had about 85-90% mark clearing success, it’s amazing and all natural ingrefients

        I just looked at the ingredient list for this product, and was disappointed to find they list ‘Natural preservative’.

        Personally I would like to see a specific ingredient name (preferably the INCI name), and legally I’m sure it’s required by the ACCC.

    Hi Zoe
    What about those of us that do have true acne scarring (not just pigment)??? I have some small, deepening craters around my jaw from severe acne as a teen and as I get older (closer to 30) they’re getting more prominent and really bothering me. All that pain as teen continues to haunt me! My skin is good now- just fighting the scarring!
    I’ve heard that certain laser treatments are good for fixing the crater look but was hoping you might be able to suggest some products/alternatives while I save for more serious treatments!

      I haven’t found any products to work that well on them and have had a number of laser treatments that made a slight difference but nothing too noticeable. I had one treatment of skin needling done and the results were amazing. Your skin improves more and more over about 2 months and even better it wasn’t expensive at all!

    After suffering a full blown staph infection I have got a lot of chicken pox style marks over my legs and arms… Hyperpigmented marks which I am really self concious about. Is there something I can use on a large scale area which isnt going to cost me an arm or leg? (Pun intended)

    I had chronic acne in my teens, still have times where it gets really bad, but I don’t have any scarring (Or post immflamatory pigmentation). I left them alone. I didnt pick, squeeze, pop, nothing.
    I was told at 15 that my skin would never recover if I didn’t do as I was told. And I am so thankful I listened.
    Not helpful advice post teen years, but those of you with preteens/ teens… Please please please tell them this. I only ever squeezed if I was in immense pain, and I washed my hands before and after.
    Acne does usually pass, treating discoloured/scarred skin is forever.

      As a skin care therapist, I absolutely agree with this advice. Also, if you have skin that is prone to breakouts you should also know that the skin will heal post inflammatory hyper-pigmentation if you get on top of the cause. Concentrate on fixing the breakout then the body will go into repair mode and sort out the pigmentation.

    Okay Zoe, quick question about the order of these products. I have been using a rosehip oil which I love but it now makes sense that it wasn’t fading any of my little marks. I also have a brightening serum so my question is – what order do I use rosehip oil, moisturiser and the serum. So confused.

      Serums it doesn’t really matter, you can layer several in order. However, with oils, I tend to use them last, just before moisturiser. (I’m not sure if there’s a scientific reason, I just do.) So, I do my brightening serum on cleansed skin, then facial oil, then cream.

    I wipe my face over with Peroxide solution ( think it’s the 30% one I get it from Coles) every second day and have done so for years. It gets rid of these marks fast and also shrinks blackheads. Might be too strong for some skin types but works for my olive skin. I am 58 and everytine I get a facial am told what great skin I have for my age and after early onset menopause brought on by chemotherapy for breast cancer and therefore not much estrogen happening this is good news.

        ProActive is one of the best-known products with benzyl peroxide in it, but it is a little bit pricey. Places like Chemist Warehouse stock Benzac, which is very similar. I used to use the Benzac 5% facial wash until my skin cleared up, and then it was too harsh once the worst of teenage skin had subsided. Now I just use the 2.5% Benzac spot cream where I need it – super effective!

        It is called Faulding Hydrogen peroxide, It says it is 3% solution not 30 -sorry. Its about 6 bucks I think. I also buy a big bag of their round face wipes or just balls of cotton wool and use that. Amazing how much dirt comes off on them after a day at work. I put moisturiser on in the morning. Works for me :)

    I reccomend the Clinique Even Better Clinical Dark Spot Corrector (must have word clinical in it, the old version doesn’t). I had good results using just one pump every night. My skintone just looked clearer, more glowy, radiant.

    My only gripe with it is that it is so bloody expensive and the tiny bottle doesnt last that long. Using only one pump at night I was lucky to get 2 months from it. I bought it on Strawberrynet for around $55 and its now more than $60 I think. Retail it is over $100.

    I wonder how much it is in UK as I am going there in 18 days!!!! Must remember to look…

      I gave up on that Clinique one for the same reason.. only using 1-2 pumps and it was gone far too quickly!
      Had better luck with Ultraceuticals Eventone serum and their clearing lotion too! HAVE A BLAST IN THE UK! I’m soooo UK-sick at the moment wish I could stow away with you!

    I suffer from hyper pigmentation and have had amazing results with the Obagi start up kit. I use with Retrieve (a prescription retin A cream) and found that all of my marks vanished within 6 weeks – I actually found it better than laser!! The bonus was the appearance of my skin – I look 10 years younger! I buy from the Internet (it’s a US brand) – well worth the effort!!!

    I got a good cream at a local compounding pharmacy – it includes hydroquinone, salycic acid, oleic acid & kojic acid. It was over-the-counter but they do a RX strength one, too (he told me to just bring the printed list of ingredients to my GP; she could call to order it for me). I paid $43 and have been using it for a few months – a little goes a long way. And it is working :-)

        Well, it is “home-made’ by the chemist – they just mix it up for you. The label says “The Fading Cream”. I imagine if you walked into any compounding pharmacy, they would have a similar formula they could mix up.

    yayyyy Zoe does it again!!! I would love to see a Zoe post about IPL too – I would definitely consider it as I’d love not to have to wear makeup to cover my teenage pimple scars. Although the overall skin toning and brightening of the products Zoe recommends sounds super appealing too.

      I’m just finishing a course of glycolic peels to target the scars I have on my chin from teenage acne and have had great results. It’s really brightened my skin and evened it out. My therapist has suggested that now I’ve finished the course I might like to have one every three months as it provides a deep exfoliation that everyday skincare finds difficult to achieve. This course of peels + diligent use of active (glycolic-based) skincare + monthly facials has got my skin looking great.

    I’ve been using a ‘mask’ of Manuka honey, a squirt of fresh lemon juice and some Bragg’s Apple Cider Vinegar and find that it’s working amazingly and is a fraction of the cost and contains far less chemicals than all the other options!

      Just want to say thank you so much for your comment. About two weeks ago my skin broke out like it had’t since I battled acne as a late teen (I’m 28 now). I’d been applying benzac (which was making my skin feel tight and dry) and had pimples at three stages… 1) new ones – both blind and with a head – really sore, 2) flaky skin (itchy and gross) where I’d popped and picked 3) red spots from old pimples. Anyway, I was at the market Fri (it’s Monday now) and spotted some Manuka honey and remembering this comment I thought ‘What the heck?’ and bought some. I applied iit as a mask which I left on for 30min and after washing off the honey, the tightness/dryness/itchiness had gone, the flaky parts had softened there wasn’t much redness. Now (three days later) I have no pimples, just a few faint red marks which can easily be covered with concealer and which I have no doubt will fade shortly. THANK YOU, THANK YOU THANK YOU!!

    Are any of these products suitable to use on children’s skin? What about Asian skin? My daughter, who turned 7 yesterday, has pigmentation/melatonin scars on her arms and legs from mosquito bites and/or undiagnosed scabies infection. I have been using a Vitamin-E type cream but the scars are not diminishing. Zoe (or anyone who knows), I’d really like to know what to use to help her repair her beautiful Chinese skin! Thanks.
    PS It would also be good idea, generally, if it specifies on this article what type of skin you are referring to, eg, Caucasian, Asian, Middle Eastern, and whether or not it makes any difference.

    Do companies like Clinique do small sample sizes, to check for reactions? I don’t want to shell out $90 for a face full of swelling.

      I am using L’Occitaine products at the moment, which are on the exxy side but they always give samples of the products… I have been using the some of the Immortelle range for the last week or so combined with my usual Ella Bache stuff & it is working a treat!

      I quite often ask for samples due to a massive allergic reaction a few yrs back to salicylic acid (rashes and breakouts for months on end…not fun!) I’ve found that if you have a good discussion with the peeps behind the counter, and explain how sensitive/allergic you are or what ingredients you’re sensitive to, they’re pretty good with samples – better than if you just ask straight out for them :) . Mecca Cosmetica and Lush will pretty happily dish out just about any product into a little sample tub for you too….tho Mecca is full of the exxy prodz.

      Sensitive skin here too, I buy the travel sizes as I find I have to use the product for a week or two to see if there is a reaction. I know Dermalogica sells almost everything in a travel/trial size.

      Yep, if you ask nicely and explain that you need some time to use a sample product due to sensitive skin, Clinique should find some samples for you. When I switched to Clinique 2 years ago, I told the girl that I get eczema and I needed about 3 weeks of using a new product before I knew it was OK. I went back to her 3 weeks later and spent quite a bit of money on their products, so it’s worth their while!

    Great advice Zoe, I currently use Estee Lauder’s Idealist Illuminating serum, which I’ve had good results with – fading larger freckles, reducing redness, evening my skintone and has even almost completely removed a burst blood vessel.
    The one area of my skin which still bothers me though is some acne scarring I have on one cheek, which has remained stubbornly there since my early twenties.
    Is there any way to clear acne scarring? *fingers & toes crossed you can answer in the affirmative!

        Hey Karen, I’ve been using the EL serum a month or two since it came out, maybe half way through last year, after getting lots of samples from the sales girl at the DJ counter.
        I remember seeing results literally within a week of just using the samples – first it was just noticing that my skin was appearing healthier, then clearer, more even and then with persistent use the radiance stage of change came and that’s when I was hooked.
        I’m my second bottle (one small, one large) and so while it costs a bit, it lasts for ages (you only need a small amount) and in the end good value for money. I just invested in the under-eye cooling illuminator too and think I’m hooked on that now too!
        I’d highly recommend going to your local EL counter to grab some samples :)

      The conservative advice is not to use hydroquinone, retin A, retinol, or sacylic acid when pregnant or breastfeeding.
      Others say it’s fine because very little is absorbed through the skin.
      I’ve just started using these products since my baby started solids and isn’t breast feeding frequently.

        Quick one for you, I always check my products under this site now as she has researched all the ingredients in many products as often a product labelled “sensitive” can actually be full of nasties.

      Hi Caris, I had a burning red rash from that product, it took a week to ease.

      I contacted the manufacturer to give them feedback and they said that around 10% of consumers had problems with the formula.
